Picrorhiza seems to have immunostimulating activity. Theoretically, picrorhiza may interfere with immunosuppressant therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the concern with pairing Picrorhiza and azathioprine. Azathioprine works by calming down your immune system, which is exactly what you need to protect a transplanted kidney or to quiet the joint inflammation of rheumatoid arthritis. Picrorhiza, on the other hand, may gently nudge the immune system in the opposite direction (it seems to have immune-stimulating activity). If that happens, it could in theory work against your medication and make it a bit less effective.

I want to be honest about the evidence, though: this is based on lab and animal research, not real-world patient cases, so it's quite theoretical. Still, because the stakes are high with a drug like this, I'd skip the Picrorhiza unless your transplant team or rheumatologist says otherwise.

About Azathioprine

What Is Azathioprine Used For?

Brand names: Imuran, Azamune

Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.

Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
