Why this applies to Temsirolimus

Temsirolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the basic idea: temsirolimus is cleared from your body by a liver enzyme called CYP3A4. The piperine in white pepper has shown, in lab dish studies only, that it can slow this enzyme down. If that happened in a real person, temsirolimus could clear more slowly and build up, which may mean stronger drug effects and more side effects, things like mouth sores, low blood counts, rash, high blood sugar, or higher cholesterol.

The good news is this is theoretical, based on test-tube data, not human cases. The sprinkle of white pepper on your dinner is very unlikely to cause trouble. Concentrated piperine supplements are a different story. Since temsirolimus is serious cancer therapy, please run any pepper supplements by your oncology team first.
