Why this applies to Vilazodone

Vilazodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kratom interacts with.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.

Here's the gist for Viibryd (vilazodone): kratom may slow down a liver enzyme called CYP2D6 that helps your body process certain medicines. If kratom blocks that enzyme, vilazodone could build up a bit more than usual, which can mean stronger effects and more side effects like nausea, diarrhea, dizziness, headache, or trouble sleeping.

There's a bigger concern here too. Both kratom and vilazodone raise serotonin in the brain, so combining them could push you toward serotonin syndrome, which can cause agitation, a racing heart, sweating, shaking, or confusion. This interaction is mostly theoretical and based on lab data plus a few case reports, not strong human proof. Still, please don't start, stop, or change anything on your own. Talk it through with your pharmacist or doctor first.

Kratom might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the thing to know if you take Viibryd (vilazodone) and are thinking about kratom. Your body clears vilazodone using a liver and gut enzyme called CYP3A4. Lab studies and a small human study suggest kratom can slow that enzyme down, mostly in the gut. If vilazodone is cleared more slowly, its levels can creep up, which may mean more of its usual side effects: nausea, diarrhea, headache, trouble sleeping, or feeling jittery.

There's a bigger worry too. Both kratom and vilazodone raise serotonin, so stacking them could push you toward serotonin syndrome (agitation, fast heartbeat, sweating, shaking, confusion). This is based mostly on theory and case reports, not strong proof, but it's worth a real conversation. Please check with your pharmacist or doctor before combining them.

A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the thing to keep in mind: Vilazodone (Viibryd) already raises serotonin levels in your brain, since that's how it lifts mood. Kratom acts on some of the same brain pathways and may also nudge serotonin up. Put the two together and you can stack too much serotonin, which is what we call serotonin syndrome. There's also a thought that kratom may slow the liver enzymes that clear Vilazodone, letting more of it build up.

To be honest, this is mostly theoretical and comes from a single case report, so I don't want to alarm you. But signs like agitation, a racing heart, shaking, sweating, or fever are worth knowing. Please don't change anything on your own. Check with your pharmacist or doctor before mixing these.

Here's the short version on combining kratom with vilazodone (Viibryd). Your body partly relies on a liver enzyme called CYP2C19 to break down vilazodone. Lab studies suggest kratom may weakly block that enzyme. If it does, a bit more vilazodone could linger in your system, which in theory might bump up its usual side effects like nausea, diarrhea, dizziness, trouble sleeping, or feeling jittery.

I want to be honest though: this concern comes only from test-tube research, and the blocking effect was weak. So it's more of a theoretical flag than a proven problem. The bigger worry, frankly, is kratom itself, since it acts on opioid receptors and isn't proven safe. Please loop in your pharmacist or doctor before mixing the two.
