A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the deal with combining kratom and Azstarys (serdexmethylphenidate/dexmethylphenidate). Both can nudge up certain brain chemicals, including serotonin. Stack them together and there's a theoretical chance of pushing serotonin too high, which is called serotonin syndrome. Watch for things like agitation, a racing heart, sweating, shaking, muscle twitching, or confusion. It's worth knowing that serdexmethylphenidate is a prodrug that converts in the gut to active dexmethylphenidate, and kratom may also slow some of the liver enzymes that clear these drugs, possibly letting more stimulant build up.
I'll be honest, this warning rests on weak, anecdotal evidence (one case report with several medicines involved), so don't panic. Still, please loop in your pharmacist or doctor before mixing these.

Azstarys is approved to treat Attention Deficit Hyperactivity Disorder (ADHD) in patients aged 6 and older, including children, adolescents, and adults. It is a central nervous system (CNS) stimulant that helps manage the inattention, hyperactivity, and impulsivity associated with ADHD.
Azstarys is not recommended for children under 6 years of age, because younger children experience higher drug exposure and more side effects — including weight loss — at the same doses used in older patients.
