Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with pairing American Hellebore and Azstarys (serdexmethylphenidate and dexmethylphenidate). This stimulant can already nudge your heart rate and blood pressure up, and at higher doses it may affect the heart's electrical timing, something doctors call the QT interval. American Hellebore is a genuinely toxic plant whose alkaloids can also disturb heart rhythm and the QT interval. Stacking these two together could raise the risk of a dangerous, irregular heartbeat called torsades de pointes, along with palpitations, dizziness, or fainting.

This warning rests on limited evidence, so it's a possibility rather than a certainty. Still, American Hellebore isn't a safe supplement to begin with. Please talk with your pharmacist or doctor before going anywhere near it.
