Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the thing about pairing ephedra with Azstarys (serdexmethylphenidate and dexmethylphenidate): both are stimulants, and stacking them is asking for trouble. Azstarys already nudges up your heart rate and blood pressure as part of how it works for ADHD. Ephedra does the very same thing, and on top of that it can affect your heart's electrical timing (what we call the QT interval). Combining them can add up, raising the chance of a racing or irregular heartbeat, palpitations, a jump in blood pressure, jitteriness, and trouble sleeping.

To be fair, the heart-rhythm evidence here is on the weaker side, but ephedra is genuinely risky and is actually banned in U.S. supplements. I'd steer clear of it entirely and chat with your pharmacist or doctor about safer options.

Azstarys is a stimulant for ADHD (the serdexmethylphenidate part is a prodrug that turns into dexmethylphenidate, the active stimulant). Ephedra is also a powerful stimulant. Putting two stimulants together is the real worry here. It's not about one blocking the other, it's that their effects can stack up.

Together, they can push your heart rate and blood pressure higher than either one alone, and may bring on a racing or pounding heartbeat, jitteriness, anxiety, trouble sleeping, or chest discomfort. This combination could meaningfully raise the risk of dangerous heart and blood pressure problems. The evidence is mostly theoretical and anecdotal, but given that ephedra is banned in U.S. supplements for safety reasons, I'd steer clear. Please talk with your pharmacist or doctor about safer options.

Azstarys (serdexmethylphenidate/dexmethylphenidate) is approved to treat Attention Deficit Hyperactivity Disorder (ADHD) in patients 6 years of age and older. It is not recommended for children younger than 6 because they experience higher drug levels in the blood and more side effects at the same doses used in older patients.
