Vilazodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the situation with pomegranate and your Viibryd. In lab dishes, pomegranate juice can slow down an enzyme called CYP2D6, which is one of the pathways your body uses to clear vilazodone. If that enzyme gets blocked, the drug could theoretically build up a bit, which might mean stronger effects or more of the usual side effects like nausea, headache, trouble sleeping, or diarrhea.

That said, I don't want to alarm you. This is based only on test-tube research, and we don't actually know if it happens in real people. Enjoying pomegranate as a fruit or an occasional glass of juice is very unlikely to cause a problem. Concentrated extracts are a bigger question mark, so check with your pharmacist or doctor before using those.

Vilazodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the quick version: your body uses a liver enzyme called CYP3A4 to break down vilazodone (Viibryd). Pomegranate contains plant compounds that can slow that enzyme down a bit in lab tests. If that happened in real life, vilazodone could build up slightly and you might notice more of its usual side effects, like nausea, diarrhea, dizziness, headache, or trouble sleeping.

The reassuring news is that human studies just don't show this panning out. When researchers gave people pomegranate juice with other CYP3A4 drugs, blood levels barely budged. So this is mostly theoretical, and enjoying pomegranate as food or juice is very unlikely to be a problem. If you plan to use concentrated pomegranate extracts, just run it by your pharmacist or doctor first.

Vilazodone is not approved for use in children or teenagers. Its safety and effectiveness have not been established in pediatric patients.
