Timolol falls under ANTIHYPERTENSIVE DRUGS — the group Pomegranate interacts with.
Consuming pomegranate juice can modestly lower blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Pomegranate juice has a mild blood pressure lowering effect of its own. Timolol is a beta blocker that already works to lower your blood pressure and slow your heart rate. So when you stack the two together, their effects can add up a little, and your numbers could dip lower than expected.
If that happens, you might feel lightheaded, dizzy, unusually tired, or wobbly when you stand up quickly. The good news is this interaction is considered moderate and is mostly theoretical, based on modest blood pressure drops in studies. Enjoying pomegranate as an occasional food or juice is unlikely to cause trouble for most people. If you're drinking it daily or using concentrated extracts, just give your pharmacist or doctor a heads up so they can keep an eye on your readings.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 2Timolol also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for timolol. Your liver clears this beta blocker partly through an enzyme called CYP2D6, and in lab dishes pomegranate has been shown to slow that enzyme down. If the same thing happened in your body, timolol could hang around a bit longer and its effects could feel stronger. With a beta blocker, that might mean a slower heart rate, lower blood pressure, dizziness, or feeling unusually tired.
The important part: this is only a theoretical concern based on test-tube work, not real people, so we honestly don't know if it matters in everyday life. Enjoying pomegranate as a fruit or a glass of juice is unlikely to cause trouble. Before using concentrated pomegranate extracts, just check with your pharmacist or doctor.

What Is Timolol Used For?
Brand names: Blocadren, Timol, Timoptic
Timolol is available in ophthalmic (eye) forms and as an oral tablet, and each form has its own approved uses. Ophthalmic formulations — including Betimol, Istalol, Timolol GFS, Timolol hemihydrate, Timolol Maleate eye drops, and Timolol Maleate Ophthalmic Gel Forming Solution — are used to lower elevated intraocular pressure (pressure inside the eye) in people with ocular hypertension or open-angle glaucoma.
Timolol Maleate tablets are used orally for three separate conditions: treating high blood pressure (alone or combined with other blood pressure medicines), reducing the risk of death and repeat heart attack in people who have survived a heart attack and are clinically stable, and preventing migraine headaches.
