Fluoxetine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the short version: your body clears fluoxetine partly through a liver enzyme called CYP2D6. In lab dish studies, pomegranate appears to slow that enzyme down. If that were to happen in real life, fluoxetine could build up a bit more than usual, which might mean stronger effects or more of its typical side effects like nausea, jitteriness, trouble sleeping, headache, or upset stomach.

The big caveat is that this is theoretical. It comes from test-tube work, not from people, so we honestly don't know if it matters in everyday life. Enjoying pomegranate as a fruit or a normal glass of juice is unlikely to be a problem. I'd just be a little more cautious with concentrated extract supplements, and it never hurts to mention it to your pharmacist or doctor.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2C9.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

Here's the short version on pairing pomegranate with your fluoxetine (Prozac): the concern on paper is that pomegranate might slow down a liver enzyme called CYP2C9, which helps break down some medicines. If that happened, fluoxetine could theoretically build up a bit, nudging up side effects like nausea, jitteriness, trouble sleeping, or headache. Fluoxetine works as-is, so this would lean toward a slightly stronger effect, not a weaker one.

But honestly, this is mostly a lab and animal idea. In actual human studies, pomegranate juice and extract didn't meaningfully change CYP2C9. So enjoying pomegranate as a food or juice is very unlikely to cause trouble. If you're thinking about concentrated extract capsules, just run it by your pharmacist or doctor first.
