CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Doxorubicin
Doxorubicin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Pomegranate interacts with.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the short version: doxorubicin is partly cleared by a liver enzyme called CYP3A4, and pomegranate has some compounds that can slow that enzyme down in the lab. In theory, blocking CYP3A4 could let doxorubicin build up a little, which might mean more of its usual side effects, things like low blood counts, mouth sores, nausea, hair loss, or added strain on the heart.
But the real-world picture is reassuring. When researchers actually tested pomegranate juice in people against other CYP3A4 drugs, it didn't meaningfully change their levels. So enjoying pomegranate as a fruit or a normal glass of juice is very unlikely to be a problem. Concentrated extracts are less studied, so I'd run those by your oncology team first. With chemo, it never hurts to ask before adding anything new.

It is given as part of combination chemotherapy for women with breast cancer that has spread to the lymph nodes after surgery, and it is used for many other cancers — from blood cancers like leukemia and lymphoma to solid tumors like bone and soft tissue sarcomas.
It is considered a broad-spectrum cancer fighter, meaning it is active against many different tumor types. It is almost always given alongside other chemotherapy drugs rather than on its own.
