Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the gist for Aristada (aripiprazole lauroxil): your body relies partly on an enzyme called CYP2D6 to clear this medication. In lab tests, pomegranate seemed to slow that enzyme down. If that happened in real life, aripiprazole could build up a bit, which might mean more of its usual effects like drowsiness, restlessness, dizziness, or mild tremor.

The honest part: this is only seen in a test tube, not in actual patients, so we really don't know if it matters for people. Enjoying pomegranate as a fruit or an occasional glass of juice is very unlikely to cause trouble. Concentrated extracts are where I'd be more cautious. Since this is a long-acting injection, just give your pharmacist or doctor a heads-up before adding supplements.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist with Aristada. Your body uses an enzyme called CYP3A4 to help clear aripiprazole, and some compounds in pomegranate can slow that enzyme down a bit. In theory, that could let aripiprazole levels creep up, which might mean more side effects like sleepiness, restlessness, lightheadedness, or a feeling of inner jitteriness (akathisia).

The good news: this is mostly a lab-based worry. When researchers tested pomegranate juice in real people on other CYP3A4 medications, it didn't move drug levels in a meaningful way. So a glass of juice or eating the fruit is very unlikely to cause a problem here. Concentrated pomegranate extracts are less studied, so it's still worth a quick mention to your pharmacist or prescriber, especially since this is a long-acting injection.

Aripiprazole injection is used to treat schizophrenia and to provide maintenance treatment for bipolar I disorder in adults. Different injectable products carry these approvals.

Abilify Asimtufii is approved for the treatment of schizophrenia in adults and as maintenance monotherapy for bipolar I disorder in adults. Aristada is approved for the treatment of schizophrenia in adults, with Aristada Initio used specifically to help start Aristada therapy.
