CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Erythromycin Stearate
Erythromycin Stearate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Pomegranate interacts with.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the short version: your body uses an enzyme called CYP3A4 to help process erythromycin, and some lab studies show pomegranate's plant compounds can slow that enzyme down. In theory, slowing it could let erythromycin build up a bit, which might mean more of its usual side effects like nausea, stomach cramping, or diarrhea. Erythromycin can also affect heart rhythm at higher levels, so that's worth a mention.
The good news is that this is mostly a theoretical worry. When researchers actually gave people pomegranate juice alongside other CYP3A4 drugs, they didn't see meaningful changes. A glass of juice or eating the fruit is very unlikely to cause a problem. If you're using concentrated pomegranate extracts, just give your pharmacist a quick heads-up to be safe.

About Erythromycin
What Is Erythromycin Used For?
Brand names: Erythro-S, Erythrocin, Ethril
Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.
Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
