Pomegranate and Enalapril Maleate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Pomegranate interacts with whole categories of medication. Enalapril Maleate falls into 2 of those categories, so it can interact with Pomegranate in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Enalapril Maleate

Enalapril Maleate falls under ANTIHYPERTENSIVE DRUGS — the group Pomegranate interacts with.

Theoretically, taking pomegranate with antihypertensive drugs might increase the risk of hypotension.
Consuming pomegranate juice can modestly lower blood pressure.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: enalapril (Vasotec, Epaned) already works to bring your blood pressure down. Pomegranate, especially as juice, can nudge blood pressure a little lower on its own. So when you pair the two, those effects can add up and your numbers may dip more than expected.

For most people enjoying pomegranate as a food or an occasional glass of juice, this is usually mild. The thing to watch for is blood pressure that runs too low, which can leave you feeling dizzy, lightheaded, or faint, especially when you stand up quickly. Concentrated extracts are more likely to matter than fruit on your plate. This is mostly theoretical and based on modest effects, so don't panic. Just check with your pharmacist or doctor before adding regular pomegranate supplements.

ACE INHIBITORS (ACEIs)

2 of 2

Enalapril Maleate also falls under ACE INHIBITORS (ACEIs).

Theoretically, taking pomegranate with ACEIs might increase the risk of adverse effects.
Pomegranate juice is thought to have ACE inhibitor-like effects.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with pomegranate and enalapril (Vasotec, Epaned). Pomegranate seems to have a mild blood-pressure-lowering, ACE-inhibitor-like effect of its own. Since enalapril is already an ACE inhibitor, stacking the two could, in theory, push your blood pressure down a little more than expected. That might show up as dizziness, lightheadedness when you stand up, or feeling unusually tired.

I want to be honest about the strength of this: it's largely theoretical and based on limited research, not strong proof. Enjoying pomegranate as an occasional food or a normal glass of juice is unlikely to cause trouble for most people. Where I'd be more cautious is with concentrated pomegranate extracts. If you notice low readings or new dizziness, check in with your pharmacist or doctor before changing anything.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.

From our pharmacist’s overview on the Enalapril Maleate page

“Potassium is the supplement I worry about first with enalapril maleate (Vasotec, Epaned). This ACE inhibitor, used for high blood pressure and heart failure, already makes the body hold onto potassium, so adding more can raise blood levels too far, a problem called hyperkalemia.”

About Pomegranate

What is Pomegranate?

Pomegranate
  • Heart and blood vessel health
  • Antioxidant support
  • High blood pressure
  • Inflammation
  • Digestive upset
  • Skin health

Pomegranate is a nutrient-rich fruit that is high in antioxidants and is widely enjoyed as food and juice. Early research suggests it may support heart health and blood pressure, but the evidence is still limited and not strong enough to call it a proven treatment. As a food it is generally safe, but talk to your pharmacist or doctor before using concentrated extracts, especially if you are pregnant, breastfeeding, or take medications.

About Enalapril

What Is Enalapril Used For?

Brand names: Vasotec, Epaned

Enalapril is used to treat high blood pressure (hypertension) in adults and children older than one month, and to treat symptomatic heart failure. Lowering blood pressure reduces the risk of stroke and heart attack.

It is also used for asymptomatic left ventricular dysfunction — when the heart's pumping ability is reduced but the person doesn't yet have obvious heart failure symptoms — to slow the progression to overt heart failure and reduce hospitalizations. Enalapril maleate oral solution (including Epaned) and enalapril maleate tablets (including Vasotec) share these three approved uses.
