Pomegranate and Hydralazine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Hydralazine

Hydralazine falls under ANTIHYPERTENSIVE DRUGS — the group Pomegranate interacts with.

Theoretically, taking pomegranate with antihypertensive drugs might increase the risk of hypotension.
Consuming pomegranate juice can modestly lower blood pressure.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: pomegranate, especially as juice, can gently nudge your blood pressure down on its own. Since hydralazine is already working to relax your blood vessels and lower your pressure, adding pomegranate could stack the two effects together. The result might be a bit more blood pressure lowering than you expect.

If your pressure drops too low, you could feel lightheaded, dizzy, tired, or notice your heart pounding (hydralazine can already cause a fast heartbeat). The good news is this effect is considered modest, and a normal amount of pomegranate as food is unlikely to cause trouble. Concentrated extracts are the bigger question mark. If you enjoy pomegranate regularly, just mention it to your pharmacist or doctor so they can keep an eye on your numbers.

About Pomegranate

What is Pomegranate?

Pomegranate
  • Heart and blood vessel health
  • Antioxidant support
  • High blood pressure
  • Inflammation
  • Digestive upset
  • Skin health

Pomegranate is a nutrient-rich fruit that is high in antioxidants and is widely enjoyed as food and juice. Early research suggests it may support heart health and blood pressure, but the evidence is still limited and not strong enough to call it a proven treatment. As a food it is generally safe, but talk to your pharmacist or doctor before using concentrated extracts, especially if you are pregnant, breastfeeding, or take medications.
