Why this applies to Buspirone

Buspirone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kratom interacts with.

Kratom might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the short version: your body relies on an enzyme called CYP3A4 to break down buspirone. Kratom appears to slow that enzyme down, at least in the gut. If buspirone isn't cleared as quickly, more of it can stay in your system, which may amp up its effects and side effects. With buspirone that usually means more dizziness, lightheadedness, drowsiness, headache, or nausea.

I'll be honest, this is mostly a theoretical concern based on lab work and pharmacology, not hard proof in people taking these two together. There was also a report where kratom plus several mood medications (buspirone included) may have nudged someone toward serotonin-related symptoms. So please don't start or stop anything on your own. Run this by your pharmacist or doctor first.

Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the concern with mixing kratom and buspirone. Both can nudge serotonin levels in your brain upward. Buspirone works by acting on serotonin signaling, and kratom's alkaloids may add to that effect. On top of that, kratom is thought to slow down some of the liver enzymes (like CYP3A4) that break buspirone down, which could let more of it build up in your system.

Together, that may raise the small risk of serotonin syndrome, which can cause agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. Honestly, this is based on a single case report and theory, not strong proof, so I don't want to scare you. Still, please check with your pharmacist or doctor before combining these two.

Buspirone tablets and capsules are used to manage anxiety disorders or to relieve anxiety symptoms in the short term. Its effectiveness has been shown in clinical trials focused on Generalized Anxiety Disorder (GAD) — a condition marked by persistent, hard-to-control worry along with physical symptoms like muscle tension, restlessness, difficulty concentrating, and sleep problems.

Buspirone has also been shown to relieve anxiety in people who have mild depressive symptoms alongside their anxiety. Everyday stress and tension, on their own, generally do not require treatment with a medicine like buspirone.
