Why this applies to Olanzapine

Olanzapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kratom interacts with.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.

Here's the gist: your body partly relies on a liver enzyme called CYP2D6 to break down olanzapine. Lab studies suggest kratom can slow that enzyme down. If that happens, olanzapine may clear more slowly and build up in your system, which can mean stronger effects and more of its usual side effects, things like heavy drowsiness, dizziness, low blood pressure when you stand, weight gain, and dry mouth or constipation.

I want to be honest about the strength of this, though. It's mostly theoretical and based on lab work, and a small dose of kratom tea didn't block the enzyme in people. Still, the doses folks actually use are higher. Kratom carries its own real risks too, so please check with your pharmacist or doctor before combining them.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that kratom extract inhibits the activity of CYP1A2. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP1A2-mediaed metabolism of ziprasidone which consequently increased systemic exposure to serotonin.

Here's the gist: your body uses a liver enzyme called CYP1A2 to clear olanzapine out of your system. Lab studies suggest kratom can slow that enzyme down. If that happens, olanzapine may not be broken down as quickly, so more of it can build up in your blood.

With higher olanzapine levels, you might notice stronger versions of its usual effects, like heavy drowsiness, dizziness, low blood pressure when standing, weight changes, or feeling foggy. I'll be honest though, this concern is theoretical and based on test-tube research, not solid human studies, so it's not a reason to panic. Still, kratom carries its own real risks. Please loop in your pharmacist or doctor before combining the two.

Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the thing to know if you take olanzapine (Zyprexa) and are curious about kratom. Olanzapine nudges serotonin activity in the brain, and kratom can do the same. Stacking the two might push serotonin too high, raising the small risk of serotonin syndrome. Watch for things like agitation, a racing heart, shivering or muscle twitching, sweating, and confusion. On top of that, kratom may slow down some of the liver enzymes (like CYP1A2) that clear olanzapine, which could let more of the drug build up and worsen side effects such as heavy drowsiness and dizziness.

I'll be honest with you: this is largely theoretical and based mostly on a single case report, so it isn't a proven danger. Still, kratom carries real risks on its own, so please loop in your pharmacist or doctor before combining them.

About Olanzapine

What Is Olanzapine Used For?

Brand name: Zyprexa

Olanzapine treats schizophrenia and the manic or mixed episodes of bipolar I disorder in both adults and adolescents (ages 13–17). It can also be used alongside valproate or lithium to treat manic or mixed episodes in adults. When combined with fluoxetine, olanzapine tablets and orally disintegrating tablets are used to treat depressive episodes associated with bipolar I disorder and treatment-resistant depression in adults.

Olanzapine intramuscular injection (short-acting) is used by healthcare providers to quickly calm acute agitation in adults with schizophrenia or bipolar I mania. Zyprexa Relprevv — a separate, long-acting intramuscular formulation available only through a restricted program — is indicated specifically for the treatment of schizophrenia in adults.
