Kratom and Imipramine

These two interact in 5 distinct ways. Each one is explained separately below.

Why Are There 5 Interactions?

Kratom interacts with whole categories of medication. Imipramine falls into 5 of those categories, so it can interact with Kratom in 5 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

1 of 5

Why this applies to Imipramine

Imipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kratom interacts with.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.

Here's the gist for imipramine. Your body relies on a liver enzyme called CYP2D6 to break this medication down. Kratom may slow that enzyme, so the worry is that imipramine could build up higher than expected in your system. Imipramine is an active drug, not a prodrug, so more of it means stronger effects, including its typical side effects like dry mouth, constipation, blurred vision, dizziness on standing, drowsiness, and a faster heartbeat.

I'll be honest, this is mostly theoretical, based on lab work and a couple of case reports with similar antidepressants, not solid human studies. Tricyclics like imipramine also carry real risks if levels climb too high. Please don't change anything on your own, just check with your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 5

Imipramine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that kratom extract inhibits the activity of CYP1A2. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP1A2-mediaed metabolism of ziprasidone which consequently increased systemic exposure to serotonin.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for imipramine (Tofranil). One of the liver enzymes that helps clear this antidepressant is CYP1A2. Lab studies suggest kratom can slow that enzyme down. If that happens in real life, imipramine could build up in your body more than expected, which may mean stronger drowsiness, dry mouth, constipation, blurry vision, dizziness, a fast or irregular heartbeat, and at higher levels, dangerous heart rhythm changes.

I want to be honest that this is theoretical, based mainly on test-tube data, not solid human studies, so we don't know how big the effect really is. Still, kratom carries its own real risks and adds to drowsiness and serotonin effects. Please loop in your pharmacist or doctor before combining the two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 5

Imipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Kratom might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for imipramine (Tofranil): your body relies partly on an enzyme called CYP3A4 to break this medication down. Kratom appears to slow that enzyme down a bit, mostly in your gut. If imipramine clears a little slower, more of it can hang around, which may nudge up its typical side effects, things like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, and a faster or irregular heartbeat. Imipramine is one of those drugs where the safe window isn't huge, so even modest changes deserve respect.

That said, this concern is theoretical, built on lab work and a couple of case reports, not solid human studies with imipramine itself. I wouldn't panic, but please don't pair these without first checking with your pharmacist or doctor.

SEROTONERGIC DRUGS

4 of 5

Imipramine also falls under SEROTONERGIC DRUGS.

Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with pairing kratom and imipramine (Tofranil). Imipramine raises serotonin in the brain, and kratom touches some of those same pathways. Putting the two together could, in theory, push serotonin too high, raising the risk of a reaction called serotonin syndrome. Warning signs include agitation, a racing heart, shivering, sweating, muscle twitching, and confusion.

There's a second worry too. Kratom may slow the liver enzymes that break imipramine down, so more of the drug can build up. That could mean stronger side effects like drowsiness, dry mouth, constipation, dizziness on standing, and heart rhythm changes. Honestly, this is based mostly on a single case report and theory, not strong proof. Still, please loop in your doctor or pharmacist before mixing them.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

5 of 5

Imipramine also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.

Theoretically, kratom might increase the levels of drugs metabolized by CYP2C19.
In vitro research shows that kratom extract weakly inhibits CYP2C19 enzyme activity.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with kratom and imipramine (Tofranil). Your body partly uses a liver enzyme called CYP2C19 to break imipramine down. In lab dish studies, kratom only weakly blocks that enzyme. If it slows things down even a little in real life, imipramine could build up slightly, which might mean more of its usual side effects: dry mouth, constipation, drowsiness, blurry vision, dizziness on standing, or a faster heartbeat.

I want to be honest though, this is theoretical and based only on lab data, not on people taking them together, so the real-world risk looks low. The bigger concern is that both kratom and imipramine affect the brain and heart on their own. Please loop in your pharmacist or doctor before combining them.
