Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that Bulbine natalensis stem extract slightly induces CYP1A2 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP1A2 substrates; however, this has not been studied in humans.

Here's the gist for imipramine (Tofranil): your liver uses an enzyme called CYP1A2 to help break this antidepressant down. In test-tube studies, Bulbine natalensis seemed to nudge that enzyme to work a little faster. If the same thing happened in people, your body could clear imipramine quicker, which might weaken how well it works and let symptoms like low mood, or bedwetting in kids, creep back.

Honestly, this is only a theory from lab data, not something proven in humans, so I wouldn't lose sleep over it. Still, imipramine isn't a medicine to fiddle with on your own. Before adding Bulbine natalensis, please check with your pharmacist or doctor, especially since this supplement has its own liver and kidney safety questions.

If that holds true in people, your body could clear the drug a little faster, which might lower its levels and weaken how well it works. Here's the basic idea: imipramine (Tofranil) is partly cleared by a liver enzyme called CYP3A4. Lab studies suggest Bulbine natalensis may speed up that enzyme. If it did the same thing in people, your body could break imipramine down a bit faster, which might lower its levels and make it work less well for your depression or your child's bedwetting.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
