Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist for prednisone fans of Bulbine natalensis. Your liver uses an enzyme called CYP3A4 to help process prednisone. In lab dish studies, Bulbine natalensis appears to speed up that enzyme. If that holds true in people, your body could clear prednisone a little faster, which might leave you with weaker results, think a flare of the joint pain, swelling, allergy symptoms, or inflammation prednisone is meant to control.

I want to be straight with you though: this is only a theoretical concern from test-tube work, never confirmed in humans. Since prednisone often treats serious conditions, and Bulbine itself raises liver and kidney safety questions, please run this combo past your pharmacist or doctor before mixing them.

Here's the honest picture: prednisone is moved around your body partly by a transport pump called p-glycoprotein. In a lab dish, Bulbine natalensis seemed to ramp up the gene for that pump. If that actually happened in people, the pump could push prednisone out faster, which might lower its levels and make it work a little less well for the condition you're treating, whether that's arthritis, lupus, asthma, or a flare-up.

I want to stress how shaky this is. This comes only from test-tube work, not from real patients, and even the study didn't confirm the pump's activity actually changed. So I wouldn't lose sleep over it. Still, given Bulbine's separate liver and kidney concerns, please check with your pharmacist or doctor before pairing them.
