Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist for hydrocortisone. Your liver uses an enzyme called CYP3A4 to help break this steroid down. In lab dishes, Bulbine natalensis seems to rev that enzyme up. If the same thing happened in your body, your hydrocortisone could be cleared a little faster, which might mean it works less well. If you take it for adrenal replacement or to control inflammation, that could show up as more fatigue, joint flare-ups, or your underlying condition not staying as calm as usual.

I want to be honest though: this is only a theoretical concern from test-tube research, never confirmed in people. Bulbine also carries its own liver and kidney safety questions. Before adding it, please run it by your pharmacist or doctor.

Here's the gist of it: hydrocortisone is moved around your body partly by a transporter called p-glycoprotein, which acts like a little pump. Lab research suggests Bulbine natalensis may speed that pump up. If that happened in people, it could push hydrocortisone out faster, possibly weakening its effect. For someone relying on steroid treatment for an arthritis flare, an allergic reaction, or low cortisol levels, a weaker effect could mean symptoms creeping back.

But I want to be honest about the strength of this. It's theoretical, based only on cell studies, not on humans, and the research didn't even confirm the pump actually changed. So I wouldn't panic. Just loop in your pharmacist or doctor before combining the two, especially since this supplement also raises some liver concerns.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
