Lidocaine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist for lidocaine: your liver clears it partly through an enzyme called CYP3A4. In lab dishes, Bulbine natalensis seemed to speed up that enzyme. If that holds true in people (and we honestly don't know yet), it could break lidocaine down a little faster and make it slightly less effective. Lidocaine isn't a prodrug, so faster clearance simply means weaker pain relief, not stronger effects.

Realistically, this matters most for injected or prescription patch lidocaine, not the small amounts in an over-the-counter cream or patch you rub on skin. This is a theoretical concern based only on test-tube data, so don't panic. Still, before pairing the two, run it past your pharmacist or doctor.

What Is Lidocaine Transdermal Patch Used For?

Brand names: Xylocaine, Dilocaine, Lidoject-1, Xylocaine MPF, Nervocaine, Lidoject-2, Duo-Trach Kit, Octocaine, Lidoderm, Ztlido

Lidocaine skin patches are applied directly to the painful area to treat post-herpetic neuralgia (PHN) — the persistent burning or aching nerve pain that can remain after a shingles outbreak.

Lidoderm (lidocaine patch 5%) and ZTlido (lidocaine topical system 1.8%) are both indicated for this use in adults. Note that despite their different strengths, one ZTlido topical system provides equivalent lidocaine exposure to one Lidoderm patch.
