Cyclosporine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with pairing these two. Your body clears cyclosporine using a liver enzyme called CYP3A4. Lab studies suggest that Bulbine natalensis may rev up that enzyme, meaning it could speed up how fast cyclosporine gets broken down. If that happens, your cyclosporine levels could drop too low, and for a transplant patient that's serious, because weaker drug levels can let your immune system start attacking the new organ (rejection). For psoriasis or arthritis, you might just notice it stops working as well.

I'll be honest with you: this is based only on test-tube research, not real people, so we don't know how strong it really is. But cyclosporine is a drug where small changes matter a lot. Please don't add Bulbine without first checking with your transplant team or pharmacist.

Here's the situation with cyclosporine and Bulbine natalensis. Cyclosporine relies on a transporter called P-glycoprotein to help move it out of your cells, and lab (test-tube) research hints that this supplement might nudge that transporter to work harder. If that happened in people, your body could push cyclosporine out faster, leaving lower levels in your blood. For a transplant patient, that's a real worry, because too little cyclosporine can let your immune system start attacking the new organ. For arthritis or psoriasis, it could simply mean the medicine works less well.

That said, this is only a theory based on lab data, not human studies, so we honestly don't know how much it matters. Cyclosporine is a narrow-margin drug where small shifts count, so please loop in your transplant team, doctor, or pharmacist before adding this.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
