Why this applies to Thioridazine

Thioridazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kratom interacts with.

Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.

Here's the concern with pairing kratom and thioridazine. Your liver uses an enzyme called CYP2D6 to clear thioridazine out of your body. Lab studies suggest kratom can slow that same enzyme down. If that happens, thioridazine can build up to higher levels than intended, which may mean more side effects like drowsiness, dizziness, a dry mouth, constipation, or a faster heartbeat.

What makes me especially cautious here is that thioridazine is well known for affecting heart rhythm (a change called QT prolongation), and higher blood levels can raise that risk. The evidence for this specific combo is still theoretical, based mostly on lab work and a few case reports with other drugs, not solid human studies. Even so, given thioridazine's heart risks, please talk with your doctor or pharmacist before mixing these.

Theoretically, kratom might increase the levels of drugs metabolized by CYP2C19.

Here's the gist for thioridazine (Mellaril): kratom has shown, in lab dish studies only, a weak ability to slow down a liver enzyme called CYP2C19. Thioridazine is partly handled by that enzyme, so in theory kratom could let a little more of it build up in your system. Thioridazine is not a prodrug, so more of it would mean stronger effects, things like heavy drowsiness, dizziness, low blood pressure, or a fast heartbeat.

That said, this is only a theoretical concern, and the effect on this enzyme looks mild. My bigger worry is that kratom acts on opioid receptors and can add to sedation, plus thioridazine already carries serious heart-rhythm risks. Please don't combine these without talking to your doctor or pharmacist first.

What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
