Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the gist: granisetron (Kytril, Sustol) works by blocking serotonin signals in your gut and brain to keep nausea and vomiting from chemo or radiation under control. Kratom also nudges your serotonin system. When you stack two things that touch serotonin, there's a theoretical worry about pushing levels too high, a rare but serious problem called serotonin syndrome (think agitation, sweating, fast heartbeat, shaking, or muscle stiffness).

I want to be honest about the strength of this: it rests on a single case report involving several other medications, not granisetron itself, so the evidence here is thin and mostly theoretical. Still, with both substances acting on serotonin, it's worth a quick chat with your pharmacist or oncology team before mixing them rather than guessing on your own.

About Granisetron

What Is Granisetron Used For?

Brand names: Kytril, Sustol

Granisetron is used to prevent nausea and vomiting linked to emetogenic (nausea-causing) cancer chemotherapy — including high-dose cisplatin — and radiation therapy, including total body irradiation and fractionated abdominal radiation. Granisetron hydrochloride tablets and Granisol oral solution are approved for both the chemotherapy and radiation indications. Sancuso skin patch is approved for prevention of nausea and vomiting in adults receiving moderately and/or highly emetogenic chemotherapy regimens of up to 5 consecutive days.

Granisetron hydrochloride injection (intravenous) is also used to prevent and treat postoperative nausea and vomiting in adults. Sustol (subcutaneous injection) is used in combination with other anti-nausea medicines in adults specifically for the prevention of acute and delayed nausea and vomiting associated with moderately emetogenic chemotherapy or anthracycline and cyclophosphamide combination chemotherapy regimens.
