Benzhydrocodone, Acetaminophen falls under CNS DEPRESSANTS — the group Kratom interacts with.
Kratom contains mitragynine, a mu-receptor agonist that may cause respiratory depression. Although results from animal research suggest that mitragynine causes less respiratory depression than the CNS depressant codeine, fatalities have been reported for patients who ingested a combination of kratom and O-desmethyltramadol, another mu-receptor agonist. Additionally, observational research suggests that adverse effects such as drowsiness and coma are more likely to occur in individuals taking kratom in combination with agents such as opioids and sedatives. Theoretically, ingesting kratom along with other CNS depressants can increase the risk of adverse effects, including drowsiness, coma, and/or severe or fatal respiratory depression.

Here's the thing to know about Apadaz (benzhydrocodone with acetaminophen): the benzhydrocodone part is an opioid that calms your brain and slows your breathing. Kratom works on those same opioid receptors in the brain. So if you stack the two, you're essentially pushing the same gas pedal twice.
That can mean stronger sedation than you expect: heavy drowsiness, confusion, very slow or shallow breathing, and in the worst cases unresponsiveness. This concern is mostly theoretical and based on how these substances act, but real-world reports of kratom mixed with opioids worsening these effects make it worth taking seriously. CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.

Here's the wrinkle with Apadaz: the benzhydrocodone part is a prodrug. Your body uses an enzyme called CYP2D6 to convert it into hydrocodone, which is the piece that actually relieves pain. Lab studies suggest kratom can block that same enzyme. If it does, less of your medicine gets switched on, so you might get weaker pain relief than expected.
At the same time, kratom itself acts on opioid receptors, so stacking it with an opioid painkiller can pile on side effects like heavy drowsiness, slowed breathing, constipation, and confusion. The enzyme data here is mostly theoretical (low-dose human studies didn't show much), but the added opioid-on-opioid risk is real. Please run this by your pharmacist or doctor before combining them.

In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP1A2-mediaed metabolism of ziprasidone which consequently increased systemic exposure to serotonin.

Here's the thing to keep in mind with Apadaz (benzhydrocodone and acetaminophen). The concern on file is that kratom may block a liver enzyme called CYP1A2 in lab studies. That said, this isn't the main pathway that handles either ingredient in Apadaz, so this particular enzyme effect is mostly theoretical and based on test-tube data, not strong human evidence.
The bigger, more real-world worry is simpler: kratom itself acts on your brain's opioid receptors, and so does this medicine. Stacking two opioid-like things together can pile up the effects, meaning more drowsiness, slowed breathing, confusion, or constipation. That combination can be dangerous. CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the thing worth knowing about pairing kratom with Apadaz (benzhydrocodone and acetaminophen). Kratom appears to mildly block an enzyme called CYP3A4, and benzhydrocodone is a prodrug, meaning your body has to convert it into active hydrocodone before it works. So this interaction could shift things in either direction, but the bigger worry is simpler: both kratom and the opioid part of Apadaz hit the same opioid receptors in your brain. Stacking them can pile on drowsiness, slowed breathing, confusion, and a dangerous drop in alertness.
This particular enzyme link is mostly theoretical, based on lab and a little human data, so I don't want to overstate it. But the opioid-on-opioid risk is real. Please talk with your pharmacist or doctor before combining these.

A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the thing to know about combining kratom with Apadaz (benzhydrocodone and acetaminophen). Kratom acts on the same opioid receptors that the benzhydrocodone part of this medicine works on. So stacking them can pile on opioid effects like heavy drowsiness, slowed or shallow breathing, confusion, and dangerously low blood pressure. That overlap is a real safety worry.
There's also a separate, more theoretical concern: kratom may slightly raise serotonin and could slow down certain liver enzymes. This is based mostly on anecdotal reports, so it's weak evidence, but combined opioid sedation is the bigger issue here. What Is Benzhydrocodone and Acetaminophen Used For?
Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
