Theoretically, berberine might increase serum levels of drugs metabolized by CYP2C9.
Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, taking berberine with drugs metabolized by CYP2C9 might increase drug levels and increase the risk of adverse effects.

Here's the gist for omeprazole and berberine. Your body uses liver enzymes (mainly CYP2C19, with some help from CYP2C9) to clear omeprazole out of your system. Berberine appears to slow down CYP2C9 a bit, so in theory it could let a little more omeprazole linger in your blood. Omeprazole isn't a prodrug, so more of it simply means a somewhat stronger effect.

In real life, this is mostly a theoretical concern based on early, lower-quality research, and omeprazole is generally a forgiving, well-tolerated medicine. If anything, you might notice mild things like headache, nausea, or loose stools (and berberine itself can upset your stomach). Nothing here means you need to stop either one, but do mention the combination to your pharmacist or doctor.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist for these two together. Your body partly clears omeprazole using a liver enzyme called CYP3A4, and berberine can mildly slow that enzyme down. When the cleanup crew works slower, a little more omeprazole can linger in your system. Since omeprazole isn't a prodrug (it's already active when you swallow it), that means the effect could be slightly stronger, not weaker.

In real life that might show up as more of omeprazole's usual side effects, like headache, nausea, gas, or diarrhea. The good news is this is mostly a theoretical concern, and omeprazole is generally forgiving. Still, before pairing berberine with it, run it by your pharmacist or doctor so they can look at your full medication list.

About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
