Kratom + Formoterol Fumarate, Glycopyrrolate
Formoterol Fumarate, Glycopyrrolate brand name: Bevespi Aerosphere.
Formoterol Fumarate, Glycopyrrolate falls into 2 drug categories that Kratom can interact with; the most serious is rated moderate. Theoretically, kratom might increase the levels and clinical effects of drugs metabolized by CYP2D6.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Kratom interacts with whole categories of medication. Formoterol Fumarate, Glycopyrrolate falls into 2 of those categories, so it can interact with Kratom in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Formoterol Fumarate, Glycopyrrolate
Formoterol Fumarate, Glycopyrrolate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kratom interacts with.
In vitro research suggests that kratom extract inhibits CYP2D6 enzyme activity. However, human research shows that a single low dose of kratom tea 2 grams does not inhibit CYP2D6 activity. However, this kratom dose is lower than what is normally taken for psychoactive effects. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP2D6-mediated metabolism of buspirone and consequently increased systemic exposure to serotonin. In another case, a 37-year-old male stable for 15 years on amitriptyline presented with anticholinergic symptoms including xerostomia, dry eyes, and constipation, along with mildly elevated bilirubin and liver enzymes after taking progressively higher doses of up to 14 grams of kratom daily for 12 weeks. It was hypothesized that the adverse effects of amitriptyline were precipitated by CYP2D6 inhibition by kratom.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Bevespi Aerosphere combines two inhaled medicines: formoterol (a long-acting bronchodilator) and glycopyrrolate. Formoterol is partly broken down by the liver enzyme CYP2D6. The concern here is that kratom may slow that enzyme down, which in theory could let a bit more formoterol build up in your system. Keep in mind this is a theoretical worry based mostly on lab studies, not strong human data, and one small study even showed low-dose kratom didn't block CYP2D6.
If levels did rise, you might notice more of formoterol's typical effects, like a faster or pounding heartbeat, shakiness, jitteriness, or trouble sleeping. Since kratom itself isn't proven safe and carries its own risks, please don't start, stop, or change anything on your own. Check with your pharmacist or doctor first.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES
2 of 2Formoterol Fumarate, Glycopyrrolate also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.
In vitro research shows that kratom extract weakly inhibits CYP2C19 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: the concern on file is that kratom may weakly slow down a liver enzyme called CYP2C19, which could let drugs cleared by that enzyme build up a bit. But this really doesn't fit your Bevespi Aerosphere well. Formoterol and glycopyrrolate are inhaled medicines, and they aren't meaningfully broken down by CYP2C19, so this particular interaction is mostly theoretical and unlikely to matter for you.
That said, I'd still be cautious. Kratom acts on the brain and can affect heart rate and blood pressure, and formoterol can already cause a fast heartbeat, jitteriness, or shakiness. Combining them could stack those effects. Since kratom carries real safety and dependence risks, please talk with your pharmacist or doctor before mixing them.
