Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the basic idea with Danshen and sildenafil (Viagra, Revatio). Your body uses a liver enzyme called CYP2C9 to help clear sildenafil out. In lab studies, compounds in Danshen seem to slow that enzyme down. If that happened in a person, sildenafil could hang around longer and build up, which may turn up its effects and side effects, things like headache, facial flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.

I want to be honest about the strength of this, though. It comes from test-tube research only, and it has not been reported in actual people, so it's theoretical for now. Sildenafil is not a prodrug, so the direction here is more drug, not less. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

Sildenafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body clears sildenafil using a liver and gut enzyme called CYP3A4. Danshen seems to nudge that enzyme in different directions depending on how you take it. In one small human study, taking danshen for 10 to 14 days actually sped up this enzyme, which could lower sildenafil levels and make it work less well (for ED, a weaker or shorter erection; for pulmonary hypertension, less benefit). Oddly, a single dose did the opposite and pushed levels up, which can mean more headache, flushing, a stuffy nose, or low blood pressure and dizziness.

Because danshen can also affect blood clotting and circulation on its own, I'd run this combo past your pharmacist or doctor before mixing them. Don't change your sildenafil on your own.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
