Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist: Danshen may gently lower blood pressure on its own. Since irbesartan (Avapro) is already working to relax your blood vessels and bring your numbers down, stacking the two together could push your pressure a little lower than intended. This isn't about how irbesartan is broken down, it's simply two things nudging in the same direction.

To be honest with you, this concern comes from animal and lab research, not solid human studies, so the evidence here is weak. Still, if your pressure dips too low you might feel dizzy, lightheaded, tired, or off-balance, especially when standing up quickly. I wouldn't change anything on your own. Just check with your pharmacist or doctor before adding Danshen, and keep an eye on your blood pressure if you do.

Irbesartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Your body relies on a liver enzyme called CYP2C9 to help clear irbesartan (Avapro). Danshen, at least in test-tube studies, seems to slow that same enzyme down. In theory, that could let irbesartan build up a bit higher than usual, nudging its blood-pressure-lowering effect stronger. You might notice things like dizziness, lightheadedness when standing, or feeling unusually tired.

I want to be honest about how soft this is: it comes from lab data only, and nothing like it has actually been reported in people. So it is a maybe, not a sure thing. Still, since danshen can also affect blood clotting, it is worth a quick chat with your pharmacist or doctor before combining the two.

What Is Irbesartan Used For?

Brand names: Avapro, Aprovel

Irbesartan tablets — sold as Avapro and under the generic name Irbesartan — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events, including stroke and heart attack. The medication can be used alone or combined with other blood pressure medicines.

Irbesartan tablets are also used to treat diabetic nephropathy (kidney disease caused by diabetes) in adults who have type 2 diabetes, high blood pressure, elevated creatinine levels, and significant protein in the urine. In these patients, it slows the worsening of kidney disease.
