Why this applies to Mirtazapine

Mirtazapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist: your body uses a liver enzyme called CYP1A2 to help break down mirtazapine. In test-tube studies, danshen seems to slow that enzyme down. If that slowdown happened in real life, mirtazapine could clear from your body a little more slowly and build up, which might mean stronger effects like extra drowsiness, dizziness, dry mouth, or increased appetite. Mirtazapine is the active drug itself (not a prodrug), so more of it means more effect, not less.

The good news is this is mostly theoretical. When researchers tested danshen with another CYP1A2 drug in people, they didn't see a real change. Still, it's worth a quick chat with your pharmacist or doctor before combining them, especially since danshen can also affect blood clotting.

Mirtazapine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your body uses a liver and gut enzyme called CYP3A4 to help break down mirtazapine. Danshen seems to push on this enzyme in a tricky way. When taken steadily for a week or two, danshen appears to speed up this enzyme, which could clear mirtazapine faster and make it work less well. That might mean your depression symptoms creep back, or sleep and appetite changes return. Oddly, a single dose of danshen seems to do the opposite and could temporarily raise levels, leading to more drowsiness, dizziness, or grogginess.

This is based on early research using a different test drug, so it's possible rather than certain. Mirtazapine isn't a prodrug, so these effects are real but modest. Please check with your pharmacist or doctor before combining them.

What Is Mirtazapine Used For?

Mirtazapine tablets, mirtazapine orally disintegrating tablets, Remeron, and RemeronsолTab are all indicated for the treatment of major depressive disorder (MDD) in adults. Major depressive disorder involves a persistently low or sad mood most days, often accompanied by loss of interest, sleep changes, fatigue, and difficulty concentrating.

The effectiveness of mirtazapine tablets in maintaining a response for up to 40 weeks after an initial treatment period has also been demonstrated. Patients should be reassessed periodically to determine whether continued treatment is needed.
