Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might decrease the levels and clinical effects of CYP2C9 substrates.
In vitro and animal research suggests that schisandra induces CYP2C9 enzymes. This effect has not been reported in humans.

Here's the gist with Schisandra and sildenafil. Your body clears sildenafil partly through a liver enzyme called CYP2C9. Lab and animal studies hint that Schisandra may speed that enzyme up, which could clear sildenafil out a little faster and make it work less well. For erectile dysfunction that might mean a weaker response, and for pulmonary hypertension (Revatio) it could mean less benefit. Sildenafil is not a prodrug, so it doesn't flip the other way.

Important: this effect has only been seen in test tubes and animals, never confirmed in people, so it's mostly theoretical. Still, if you take both, it's worth a quick chat with your pharmacist or doctor.

Sildenafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear sildenafil out of your system. Schisandra appears to slow that enzyme down, so the drug can hang around longer and reach higher levels than usual. In plain terms, you may get a stronger dose effect than you bargained for.

For sildenafil, that could mean more of its usual side effects, like headache, flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure. That last one matters most if you take it for pulmonary hypertension or alongside any blood pressure or nitrate medicines. Keep in mind this is based mostly on lab and animal data, not strong human studies. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

About Sildenafil

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
