Lansoprazole, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Prevpac is a combination pack used to treat H. pylori stomach infections, and one part of it, lansoprazole, is partly cleared by a liver enzyme called CYP2C19. In lab and animal studies, schisandra appears to slow this enzyme down. So in theory, schisandra could let lansoprazole build up a bit, which might mean more of its usual effects like headache, diarrhea, or stomach upset. Lansoprazole is not a prodrug here, so more drug means stronger effect, not weaker.

The good news is this has only been seen in test tubes and animals, never confirmed in people, so it is more of a caution than a real-world warning. Since you are taking Prevpac for a short, important course, I would just mention the schisandra to your pharmacist or doctor and let them decide.

Lansoprazole, Amoxicillin, Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

The Prevpac kit combines three medicines to treat H. pylori stomach ulcers. The one that matters most here is clarithromycin, which your body breaks down using a liver enzyme called CYP3A4. Schisandra appears to slow that enzyme down, so clarithromycin (and to a lesser extent lansoprazole) could build up a bit higher than usual. None of these are prodrugs, so more drug in your system means stronger effects and a greater chance of side effects like nausea, an upset stomach, a strange or metallic taste, or diarrhea. Amoxicillin isn't affected this way.

Keep in mind this is mostly lab and animal evidence, not strong human proof, so the real-world impact may be small. Still, it's worth a quick chat with your pharmacist or doctor before adding schisandra.
