Why this applies to Lacosamide

Lacosamide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might decrease the levels and clinical effects of CYP2C9 substrates.
In vitro and animal research suggests that schisandra induces CYP2C9 enzymes. This effect has not been reported in humans.

Here's the short version: your seizure medicine, lacosamide (Vimpat), is partly cleared by a liver enzyme called CYP2C9. Lab and animal studies hint that schisandra may speed that enzyme up. If that happened in people, your body could clear lacosamide a little faster, which in theory might lower its blood levels and make it work less well. With a seizure medicine, the worry there is breakthrough seizures.

I want to be honest about how soft this is. This effect has only shown up in test tubes and animals, never confirmed in humans, so it's more a flag than a proven problem. Lacosamide is not a prodrug, so this isn't about extra side effects. Still, since seizure control is too important to gamble with, please run schisandra past your pharmacist or doctor before starting it.

Lacosamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the gist for Vimpat (lacosamide): your body partly clears it using a liver enzyme called CYP3A4. Schisandra tends to slow that enzyme down, so in theory it could let a bit more lacosamide build up in your system than usual.

If that happened, you might notice more of lacosamide's typical side effects, things like dizziness, drowsiness, blurred or double vision, headache, or feeling unsteady on your feet. That said, I want to be honest with you: this concern comes mainly from lab and animal data, not from real-world studies in people taking lacosamide, so it stays fairly theoretical. Lacosamide is not a prodrug, so the direction is straightforward (more enzyme blocking means potentially stronger effects). Since seizure medicines are nothing to gamble with, please run this by your pharmacist or neurologist before adding schisandra.

About Lacosamide

What Is Lacosamide Used For?

Brand names: Vimpat, Motpoly XR

Lacosamide is used to treat partial-onset seizures (seizures that start in one part of the brain) in adults and children. The age at which it is approved depends on the specific product: Vimpat tablets, oral solution, and injection are approved for partial-onset seizures in patients as young as 1 month old; most other formulations are approved from age 4 and up; and Motpoly XR is approved for patients weighing at least 50 kg.

Lacosamide is also used as adjunctive (add-on) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain) in adults and children 4 years of age and older, including Motpoly XR for patients weighing at least 50 kg.
