Why this applies to Galantamine

Galantamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the simple version: your body uses a liver enzyme called CYP3A4 to help clear galantamine. Schisandra tends to slow that enzyme down. When the enzyme works slower, galantamine can hang around longer and build up a bit higher than expected.

Because galantamine is not a prodrug, more drug in your system means a stronger effect, and that often shows up as more side effects. With galantamine think nausea, vomiting, diarrhea, dizziness, loss of appetite, or a slower heartbeat. Most of the evidence here comes from lab and animal work, not real patients, so this is more of a caution than a sure thing. Still, before mixing schisandra with galantamine, please check with your pharmacist or doctor first.

Galantamine — available as extended-release capsules, tablets, and an oral solution — is used to treat mild to moderate dementia caused by Alzheimer's disease. All supported formulations share this indication.

It does not cure or slow the underlying disease, but it can help support cognitive function in people with early to moderate-stage Alzheimer's. Its benefits may lessen as the disease progresses and more brain cells are lost.
