Rabeprazole Sodium falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Your body uses an enzyme called CYP2C19 to help clear rabeprazole (Aciphex) out of your system. Schisandra, at least in lab and animal studies, appears to slow that same enzyme down. In theory, that could let rabeprazole build up a bit higher than usual, which might mean slightly stronger effects or a touch more of its typical side effects like headache, nausea, or stomach upset.
Here's the reassuring part: this interaction has only been seen in test tubes and animals, never reported in people, and rabeprazole is generally a forgiving, well-tolerated medicine. So it's worth a mention, not a worry. Still, before you add a schisandra supplement, have a quick chat with your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the gist: your body clears rabeprazole (Aciphex) partly through a liver enzyme called CYP3A4, and Schisandra appears to slow that enzyme down. When that pathway gets blocked, a bit more rabeprazole can linger in your system than usual. In practice that could mean slightly stronger effects, and possibly more of the typical complaints people get with this medicine, like headache, nausea, gas, or diarrhea.
The good news is this interaction is mostly based on lab and animal research, not solid human studies, and rabeprazole is generally a forgiving, well-tolerated drug. So I wouldn't lose sleep over it. Still, don't start or stop anything on your own. A quick chat with your pharmacist or doctor is the smart move, especially if you plan to take Schisandra regularly.

What Is Rabeprazole Used For?

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.
In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
