Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down acalabrutinib (Calquence) and clear it out. Schisandra appears to slow that enzyme down, so the medication can build up higher than expected. Acalabrutinib is not a prodrug, so more drug in your system means a stronger effect, and that can mean more side effects like bruising or bleeding, headaches, low blood counts, infections, or an irregular heartbeat.

To be fair, this is based mostly on lab and animal data, not solid human studies with acalabrutinib specifically, so think of it as a reasonable caution rather than a proven problem. Since this is a cancer therapy, please don't add schisandra on your own. Run it by your oncology team or pharmacist first.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the gist: your body uses a kind of cellular bouncer called P-glycoprotein to push acalabrutinib (Calquence) back out of your gut and limit how much gets absorbed. Schisandra appears to block that bouncer, so in theory a bit more of your medicine could end up in your bloodstream. With a targeted cancer drug like this, higher levels could mean more of its usual side effects, things like bruising or bleeding more easily, headaches, fatigue, diarrhea, or a greater risk of infection.

One important reminder: this concern comes mostly from lab and animal studies plus one small human study using a different drug, so it is more theoretical than proven. Still, since you're on cancer treatment, please don't add schisandra without first checking with your oncologist or pharmacist.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
