Avatrombopag Maleate falls under P-GLYCOPROTEIN SUBSTRATES — the group Schisandra interacts with.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the quick version: avatrombopag (Doptelet) is partly carried by a gut transporter called P-glycoprotein, which normally pumps some of the drug back out before it gets absorbed. Schisandra appears to slow that pump down, so in theory more avatrombopag could end up in your bloodstream than expected.

If that happens, you might see a stronger effect than intended. Since this drug raises your platelet count, too much push in that direction could mean platelets climbing higher than your doctor planned, which can add to clotting risk, along with possible headache, tiredness, or swelling. To be fair, most of this comes from lab and animal studies plus one small human study on a different drug, so the real-world effect on avatrombopag isn't proven. Still, please run it by your pharmacist or doctor before combining them.

About Avatrombopag

What Is Avatrombopag Used For?

Brand name: Doptelet

Avatrombopag treats thrombocytopenia (a low platelet count that raises bleeding risk) in three groups of patients. Doptelet tablets are used in adults with chronic liver disease who need their platelet count raised before a medical procedure, and in adults with chronic immune thrombocytopenia (ITP) whose platelet count has not responded well enough to earlier treatments. Doptelet tablets are also used in children aged 6 years and older with persistent or chronic ITP who have not responded to prior treatment.

Doptelet Sprinkle (oral granules) is specifically indicated for children aged 1 year to less than 6 years with persistent or chronic ITP who have not responded adequately to a previous treatment.
