Osimertinib Mesylate falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Tagrisso (osimertinib): your body uses a tiny pump called p-glycoprotein to help move this drug around and clear some of it out. Danshen may slow that pump down. In a small study, danshen raised blood levels of another pump-dependent drug by roughly a third. So the concern is that danshen could nudge your osimertinib levels a bit higher than expected.

With more osimertinib hanging around, you might notice more of its usual side effects, things like diarrhea, mouth sores, dry or itchy skin, nail changes, or fatigue. This isn't a sure thing, and the evidence is fairly limited, but osimertinib is an important cancer medicine, so don't add danshen on your own. Please run it by your oncology team or pharmacist first.

Osimertinib Mesylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your body uses an enzyme called CYP3A4 to break down osimertinib (Tagrisso), and Danshen can stir up that same enzyme. The tricky part is that Danshen pushes in two directions. In a small human study, taking it for 10 to 14 days actually sped up this enzyme, which could lower osimertinib levels and, in theory, make your cancer treatment work less well. A single dose did the opposite and raised drug levels.

Since osimertinib is an active drug (not a prodrug), lower levels mean less benefit, while higher levels could mean more side effects like diarrhea, rash, or mouth sores. This is a cancer medicine where steady levels really matter, so please don't add Danshen without first checking with your oncologist or pharmacist.

About Osimertinib

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
