Opicapone falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Opicapone (Ongentys): it's carried around your body partly by a transporter called P-glycoprotein. Danshen seems to slow that transporter down a bit, based on a small study where it raised blood levels of a similar drug. The same thing could nudge your opicapone levels up a little.

In practical terms, more opicapone (and the levodopa it boosts) could mean stronger Parkinson's side effects, like extra involuntary movements (dyskinesia), low blood pressure when you stand up, dizziness, nausea, or vivid dreams. This is a possible, moderate concern based on limited evidence, not a sure thing. Concentrated Danshen supplements are more likely to matter than tiny culinary amounts. Please check with your pharmacist or doctor before combining them.

It is not used alone. Ongentys works as an add-on to help levodopa stay active longer, reducing the time patients spend in these 'off' states.
