Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist: amlodipine already relaxes your blood vessels to bring your blood pressure down. Danshen, in early animal studies, seems to lower blood pressure too. Put them together and the two effects could add up, so your pressure may dip more than expected. With amlodipine specifically, that can show up as dizziness, lightheadedness (especially when you stand up), flushing, headache, or swollen ankles.

I want to be honest about the evidence here: this is theoretical and based on lab and animal research, not solid human studies, so it's a maybe, not a sure thing. Still, since both can nudge blood pressure the same direction, it's worth a quick chat with your pharmacist or doctor before combining them. Don't stop or change your amlodipine on your own.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for you. Your blood pressure medicine, amlodipine, is cleared from your body by a liver and gut enzyme called CYP3A4. Danshen can nudge that enzyme in either direction. In one small study, taking Danshen daily for a week or two actually sped up this enzyme, which could mean your body clears amlodipine a bit faster. That might make it work less well, so your blood pressure or chest pain may not be as well controlled.

Interestingly, a single dose of Danshen seemed to do the opposite, briefly slowing clearance and possibly boosting amlodipine's effects (think more swelling in the ankles, dizziness, or flushing). The evidence here is early and modest. Amlodipine is not a prodrug, so this is mostly about how much stays active. Please check with your pharmacist or doctor before combining them.

Theoretically, taking danshen in combination with amlodipine may decrease the clinical effects of amlodipine.
In animal research, taking danshen orally in combination with amlodipine reduced blood levels of amlodipine by about 52%. This may have been due to induction of cytochrome P450 3A4 (CYP3A4) by danshen, which has been demonstrated in vitro. So far, this interaction has not been reported in humans.

Here's the gist for amlodipine (the active ingredient in Katerzia). Your body clears amlodipine using a liver enzyme called CYP3A4. Danshen appears to speed up that enzyme, which means it could clear amlodipine out faster and leave less of it working in your system. Amlodipine is not a prodrug, so faster clearance simply means a weaker effect.

In plain terms, your blood pressure or chest pain might not be as well controlled as usual. The honest part: this comes only from lab and animal studies (one animal study saw blood levels drop about half), and it hasn't been confirmed in people. So I wouldn't panic, but I also wouldn't ignore it. Please check with your pharmacist or doctor before combining the two, and keep an eye on your blood pressure readings.

What Is Amlodipine Used For?

Brand name: Katerzia

Amlodipine treats high blood pressure (hypertension) in adults and children aged 6–17 years. Lowering blood pressure reduces the risk of strokes and heart attacks. Sdamlo (oral solution) is specifically indicated for hypertension in adults and pediatric patients 6 years of age and older.

Amlodipine also treats coronary artery disease in adults, including chronic stable angina (chest pain with exertion), vasospastic angina (Prinzmetal's or variant angina, caused by coronary artery spasms), and documented coronary artery disease in patients without heart failure or significantly reduced heart pumping function.
