Acetaminophen, Dextromethorphan falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for your Tylenol Cough Extra Strength, which pairs acetaminophen (a pain and fever reducer) with dextromethorphan (a cough suppressant). Lab tests suggest Danshen may block an enzyme called CYP1A2 that helps the body process certain medicines. In theory, that could let these drugs build up a bit and hang around longer, which might mean more drowsiness or dizziness from the dextromethorphan.

The honest truth, though, is that this is only seen in test-tube and animal studies. When researchers tested it in real people using another CYP1A2 drug, nothing meaningful happened. So this is more of a theoretical caution than a proven problem. Still, it's smart to mention any Danshen use to your pharmacist or doctor, especially since Danshen can also affect blood clotting.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

This combo cough and cold product has two parts. The dextromethorphan piece is partly handled by a liver and gut enzyme called CYP3A4, which is the enzyme Danshen can affect. Here's the catch: research suggests that taking Danshen for a week or two actually speeds up CYP3A4, which could lower dextromethorphan levels and make it less effective at quieting a cough. But a single dose of Danshen seems to do the opposite, raising levels briefly, which might bring more drowsiness or dizziness.

The good news is this is a moderate, somewhat unpredictable interaction, and the acetaminophen portion isn't really involved here. For short-term cough relief, it's usually minor, but check with your pharmacist before pairing them.

Here's the gist for your Tylenol Cough product. Part of how your body clears acetaminophen goes through a liver enzyme called CYP2E1. In test-tube studies, Danshen seems to slow that enzyme down. If that happened in people, it could in theory shift how acetaminophen is processed. The good news is this has only been seen in the lab, never reported in real patients, so I wouldn't lose sleep over it.

The piece I'd actually keep an eye on is Danshen's effect on blood clotting, plus making sure your total acetaminophen stays under safe daily limits. This isn't a reason to stop either one, but it's worth a quick chat with your pharmacist before combining them, especially if you take blood thinners.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the gist: your body clears acetaminophen partly through a process called glucuronidation, where liver enzymes attach a tag that helps flush the drug out. Danshen, in lab dishes, seems to tug on those enzymes in both directions, sometimes speeding them up and sometimes slowing them down. Because of that, it's hard to predict which way things would go in a real person. Dextromethorphan isn't a prodrug here, so we're mainly talking about acetaminophen handling.

The honest truth is this is theoretical, based only on test-tube and animal work, and it hasn't actually been seen in people. So I wouldn't lose sleep over an occasional dose of Tylenol Cough. Still, if you use Danshen regularly, just give your pharmacist a heads-up before combining them.
