Why this applies to Fenfluamine

Fenfluamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for Fintepla (fenfluramine). Your body partly clears this seizure medicine using a liver enzyme called CYP1A2. Danshen, in test-tube studies, seems to slow that enzyme down. If that played out in real life, fenfluramine could build up a bit, which might mean stronger effects or more side effects like drowsiness, loss of appetite, tiredness, or changes in heart rate or blood pressure.

The good news: this is theoretical. It comes from lab work, and when researchers tested a similar CYP1A2 drug in actual people, danshen didn't really change its levels. So this is more a caution than a proven problem. Since fenfluramine is closely monitored, please check with your child's neurologist or pharmacist before adding danshen.

What Is Fenfluramine Used For?

Brand name: Fintepla

Fintepla (fenfluramine oral solution) is approved to treat seizures associated with Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older. It is used alongside other antiseizure medications.

Both conditions are severe, often hard-to-control forms of epilepsy that typically begin in childhood. Fintepla is available only through a restricted program called the Fintepla REMS because of serious heart-related risks.
