Why this applies to Clomipramine

Clomipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the basic idea with Clomipramine (Anafranil): your liver uses an enzyme called CYP1A2 to help break it down. Danshen may slow that enzyme a bit, so in theory your body could clear clomipramine a little more slowly, letting levels creep up. Clomipramine is not a prodrug, so more drug hanging around means a stronger effect and more chance of side effects like drowsiness, dry mouth, constipation, dizziness, a faster heartbeat, or feeling shaky.

I want to keep this in perspective. This concern comes only from lab studies, and when researchers tested a similar CYP1A2 drug in real people, danshen didn't change much. So the risk is theoretical, not proven. Still, please run it by your pharmacist or doctor before combining them.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the idea with Danshen and your clomipramine (Anafranil). Lab tests suggest Danshen can slow down a liver enzyme called CYP2C9, which is one of the pathways your body uses to break clomipramine down. If that pathway is blocked, the medicine could clear out more slowly and build up in your system, which may mean stronger effects and more side effects like drowsiness, dry mouth, constipation, dizziness, a fast or irregular heartbeat, or feeling extra sedated.

I want to keep this in perspective, though. This is only seen in test-tube and animal research, and it has never actually been reported in real people. So it's more of a theoretical caution than a proven problem. Still, since clomipramine can affect heart rhythm, it's worth a quick chat with your pharmacist or doctor before combining the two.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the deal with combining Danshen and Clomipramine. Your body partly uses an enzyme called CYP3A4 to break down clomipramine, and Danshen can nudge that enzyme in either direction depending on how long you've been taking it. In one small study, taking Danshen daily for a week or two sped this enzyme up, which could lower clomipramine levels and make it work less well (think more obsessive-compulsive symptoms creeping back). But a single dose did the opposite and raised levels, which could mean more side effects like drowsiness, dry mouth, constipation, or a fast heartbeat.

Honestly, this is based on limited research using a different drug, so it's more of a heads-up than an alarm. Clomipramine isn't a prodrug, so think of it as levels going up or down. Please check with your pharmacist or doctor before adding Danshen.
