Cyproterone Acetate, Ethinyl Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Diane-35 combines an estrogen with cyproterone, and part of how your body clears these hormones runs through a liver enzyme called CYP1A2. The thinking here is that Danshen may slow that enzyme down a bit. If that happened, slightly more hormone could linger in your system, which in theory might nudge up the usual estrogen-type side effects like nausea, breast tenderness, headaches, mood changes, or bloating.

Here's the reassuring part: this is based only on lab and test-tube work. When researchers actually tested Danshen against a real CYP1A2 medicine in people, they didn't see a meaningful effect, so this is mostly theoretical. Still, since Danshen can also affect blood clotting, it's worth a quick chat with your pharmacist or doctor before combining the two.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the simple version: Danshen seems to slow down a transporter in your body called P-glycoprotein, which helps move certain drugs along. Your pill, Diane-35 (cyproterone acetate plus ethinyl estradiol), is one of those drugs that this transporter handles. When the transporter is partly blocked, a bit more of the medicine can stick around in your system instead of being shuttled out.

In plain terms, that could mean slightly higher hormone levels, and possibly more of the usual side effects like nausea, breast tenderness, headaches, mood changes, or fluid retention. This is based on limited human research (a study used a different drug, not your pill), so it's a possible concern rather than a sure thing. Please don't stop or change anything on your own. Just check with your pharmacist or doctor before adding Danshen.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to know about Diane-35: both the cyproterone and the ethinyl estradiol in it are cleared by a liver and gut enzyme called CYP3A4. The early research we have suggests that taking Danshen for a couple of weeks can speed up that enzyme, which would push these hormones out of your body faster and lower their levels.

Why does that matter? Lower hormone levels could mean weaker birth control protection and, if you're using it for acne or unwanted hair, possibly a return of those symptoms. You might also notice some breakthrough spotting. This evidence is fairly preliminary, so it's not a sure thing, but pregnancy prevention is too important to gamble on.
