Lemongrass and Mitapivat

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Mitapivat

Mitapivat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lemongrass interacts with.

Theoretically, lemongrass might decrease the metabolism of CYP3A4 substrates.
Animal research shows that lemongrass and its constituent citral inhibit CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Pyrukynd (mitapivat): your liver uses an enzyme called CYP3A4 to break this medication down. In lab and animal studies, lemongrass — mainly its compound citral — seems to slow that same enzyme. If that holds true in people, your body could clear mitapivat a little more slowly, letting levels creep up. That might mean stronger drug effects or more side effects, like headache, nausea, back pain, or changes in your lab numbers.

I want to be honest though — this is only theoretical, based on test-tube and animal work, not real human studies. The lemongrass you'd use in cooking or an occasional cup of tea is very unlikely to matter. Concentrated supplements or the essential oil are the bigger question. Before adding those, just run it by your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

Lemongrass is a fragrant tropical grass widely used as a cooking herb, a tea, and an aromatherapy oil. About Mitapivat

What Is Mitapivat Used For?

Brand name: Pyrukynd

Mitapivat is available as two separate branded products approved for different conditions. Pyrukynd is used to treat hemolytic anemia — a type of anemia caused by red blood cells breaking down too quickly — in adults who have pyruvate kinase (PK) deficiency, a rare inherited blood disorder. Aqvesme is used to treat anemia in adults with alpha- or beta-thalassemia, inherited conditions that affect how the body makes hemoglobin.

These are distinct approvals. Pyrukynd and Aqvesme are not interchangeable, and each is approved only for its specific indication.
