Rabeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the basic idea with Danshen and rabeprazole (Pariet). Your body clears rabeprazole partly through a liver enzyme called CYP2C9. In lab and animal studies, compounds in Danshen appear to slow that enzyme down. If that holds true in people, rabeprazole could build up a bit and hang around longer, which might mean stronger or more frequent side effects like headache, stomach upset, or diarrhea.

I want to be honest about the strength of this, though. So far it's only been seen in test tube and animal research, not in actual patients, so it's theoretical at this point. Rabeprazole is also a pretty forgiving medicine, so this isn't an alarming combo. Still, before adding Danshen, run it by your pharmacist or doctor so they can keep an eye on how you feel.

Rabeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: rabeprazole (Pariet) is partly cleared by a liver and gut enzyme called CYP3A4, and Danshen can nudge that enzyme. When taken regularly for a couple of weeks, Danshen tends to speed up this enzyme, which could lower your rabeprazole levels and make it work a little less well. In practice that might mean your heartburn or reflux isn't as well controlled. Oddly, a single dose of Danshen can briefly do the opposite and raise levels.

The evidence here is limited and mostly from short studies, so don't panic. Rabeprazole also leans heavily on a different enzyme (CYP2C19), so the real-world effect may be small. Still, if you're considering Danshen, please check with your pharmacist or doctor first.

What Is Rabeprazole Used For?

Brand name: Pariet

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
