Why this applies to Rabeprazole

Rabeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: your body uses liver enzymes (one called CYP2C9 is part of the picture) to clear rabeprazole out of your system. Quercetin, in concentrated supplement doses, can slow that enzyme down a bit. In theory, that means a little more rabeprazole could linger in your blood, which might nudge up the usual side effects like headache, stomach upset, gas, or diarrhea.

Honestly, though, this is mostly a theoretical concern. The hard evidence comes from other drugs, not rabeprazole itself, and the supplement amounts used were high. Quercetin from foods like onions and apples is very unlikely to matter. If you're taking high-dose quercetin capsules alongside your rabeprazole, just give your pharmacist or doctor a heads-up so they can keep an eye on how you feel.

Rabeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist for rabeprazole (Pariet). Your body partly clears this acid-reducing drug using a liver enzyme called CYP3A4. Quercetin can nudge that enzyme, but the lab and animal studies pull in both directions: sometimes it slows the enzyme (which could let rabeprazole linger and slightly raise side effects like headache or stomach upset), and in at least one human study it actually sped the enzyme up (which could mean a bit less acid control and more breakthrough heartburn).

The honest truth is this is mostly theoretical and based on weak evidence, not solid human data on rabeprazole itself. Food amounts of quercetin almost certainly won't matter. If you're considering high-dose supplements, just run it by your pharmacist or doctor first.

What Is Rabeprazole Used For?

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
