Acetaminophen, Pseudoephedrine, Triprolidine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

This combo product (Actifed Plus ES) blends acetaminophen for pain and fever, pseudoephedrine as a decongestant, and triprolidine, an antihistamine. The concern on file is that Danshen may slightly slow down a liver enzyme called CYP1A2. If that enzyme works less, drugs it handles could build up a bit and hit you harder. In theory, you might notice more drowsiness or dry mouth from the antihistamine, or extra jitteriness, a racing heart, or trouble sleeping from the decongestant.

Here's the reassuring part: this is only seen in lab and test-tube studies, and in actual people Danshen did not meaningfully change a similar CYP1A2 drug. So real-world risk looks low. Still, since Danshen can also affect bleeding, it's worth a quick chat with your pharmacist before combining them.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2E1. So far, this interaction has not been reported in humans.

Here's the piece that matters in this combo product: the acetaminophen (the Tylenol part). Your liver uses an enzyme called CYP2E1 to break a small amount of acetaminophen into a byproduct that can stress the liver. In lab and animal studies, compounds in Danshen seem to slow that same enzyme down. In theory, that could shift how acetaminophen is processed. Honestly, what this means in a real person isn't clear, and it has never actually been reported in humans, so I wouldn't lose sleep over an occasional cold tablet.

The pseudoephedrine and triprolidine pieces aren't really involved here. The bigger common-sense point is just to avoid stacking acetaminophen from several products and to skip heavy alcohol. If you take Danshen regularly, run it by your pharmacist or doctor first.

Acetaminophen, Pseudoephedrine, Triprolidine also falls under GLUCURONIDATED DRUGS.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

If you take Actifed Plus ES, the part of this combo to focus on here is acetaminophen (the pain and fever reliever, also known as Tylenol). Your body clears acetaminophen partly through a process called glucuronidation. Danshen has been shown in test tube and animal studies to tug on those same clearing enzymes, sometimes slowing them down and sometimes speeding them up. In theory, if it slows things down, acetaminophen could linger a bit longer, which is worth respecting since this is the drug most tied to liver strain at higher doses.

Here is the reassuring part: this has never actually been reported in people, only in lab research, so it is more of a caution flag than a known problem. The pseudoephedrine and triprolidine pieces are not really affected this way. Still, before pairing Danshen with this medicine, run it by your pharmacist or doctor.
