Amlodipine Besylate, Benazepril falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Lotrel is really two blood pressure medicines in one pill: amlodipine, which relaxes and widens your blood vessels, and benazepril, an ACE inhibitor that also lowers blood pressure. Danshen seems to have its own mild blood-pressure-lowering effect, so stacking it on top could push your numbers down a little more than expected. In animal studies, Danshen even seemed to boost the effect of captopril, a close cousin of benazepril.

If pressures drop too low, you might feel dizzy, lightheaded, tired, or get a headache, especially when standing up quickly. To be fair, this is mostly theoretical and based on lab and animal work, not strong human studies. Still, it's worth mentioning to your pharmacist or doctor before combining them, particularly if you already run low.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Amlodipine Besylate, Benazepril also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Lotrel combines two medicines: amlodipine (a blood pressure pill that your liver clears using an enzyme called CYP3A4) and benazepril (which works differently). The amlodipine piece is the part that matters here. With repeated daily use over a week or two, Danshen seems to speed up CYP3A4 in the gut, which could clear amlodipine faster and make it work a little less. That might show up as blood pressure creeping back up. Interestingly, a single dose of Danshen does the opposite and can raise levels, which could mean more dizziness, flushing, or ankle swelling.

This is based on early, small human research, so it is possible but not certain. Please check with your pharmacist or doctor before mixing the two, and keep an eye on your blood pressure.
