Why this applies to Aripiprazole Lauroxil
Aripiprazole Lauroxil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Aristada (aripiprazole lauroxil). This is a long-acting injection, and your liver and gut rely partly on an enzyme called CYP3A4 to clear it from your body. In a small study, taking Danshen for about 10 to 14 days sped that enzyme up, which could lower aripiprazole levels over time. For a shot you get every few weeks to keep symptoms steady, weaker coverage could mean your schizophrenia or bipolar symptoms creep back. Interestingly, a single dose of Danshen did the opposite and raised levels.
The evidence here is limited and a bit mixed, so I wouldn't panic. But since this injection is meant to give steady protection, please run any Danshen use by your prescriber or pharmacist first.
